You probably want to disable the Menu button.

If this is a common operation, rather than press the individual buttons, would it be useful to assign all three button presses to a single button macro?

Not really. The alt button inverts the the command what you do with ctrl + shift
while CTRL + SHIFT are down you are in marquee selection mode when selected triangles remain visible. If you press also press the ALT and you do same marquee selection what you get is the triangles that were outside of the marquee. So alt inverts the result what the tool does.
pressing CTRL + SHIFT and + ALT puts you in marquee mode that is live only when those buttons are pressed. Making it as a macro would break the rhythm how you use this feature just like Vaell said.
But disabling the menu button seems to do the trick perfectly!
