I've got the Spaceball 5000USB and Blender 249.2.
For whatever reason the A,B,C, and the #9 buttons won't stay mapped. As a matter of fact, they keep changing to some other random buttons. I've customized four simple items...the letters "S", "X", "Y", and "Z". They won't stick when I reopen Blender.
My other custom buttons stick...just not those four.
I've tried saving, save-as, closing Blender while mapping, etc, etc.
Anybody know the solution?

I haven't heard of this problem until now. I have Blender 2.49b and connected a SpaceBall 5000 USB. I did the same button mapping as you said - with no problems. When I stop and restart just the application everything is there as it should be. The same applies when I stop and restart the driver and then restart Blender. All button mappings I did are still there.
What is the serial number of the device?
When you look at the "About" Window of the driver (Help -> About) what version numbers do you get there?
